Eli Lilly's Michael Kopach Says Retatrutide Leads in Obesity Treatment and CMC Innovation

Lilly's SVP of Global Drug Product Manufacturing outlines how retatrutide's triple-agonist mechanism and manufacturing innovation position it ahead of current GLP-1 therapies.

Eli Lilly senior executive Michael Kopach has signalled the company’s confidence in retatrutide’s potential to redefine obesity treatment, highlighting both the drug’s unique triple-agonist mechanism and the manufacturing innovation underpinning its development.

Kopach, Lilly’s Senior Vice President of Global Drug Product Manufacturing, told BioXconomy that retatrutide represents a substantial advancement over existing incretin-based therapies, citing its simultaneous targeting of GIP, GLP-1, and glucagon receptors. The combination drives greater weight loss than single- or dual-agonist approaches, as demonstrated in Phase 2 trials (24.2% mean weight loss at 48 weeks) and the recent TRIUMPH-1 Phase 3 readout (30.3% mean weight loss).

On the manufacturing side, Kopach emphasized Lilly’s investment in Chemistry, Manufacturing, and Controls (CMC) innovation — the regulatory framework that ensures every dose of an injectable drug meets consistent quality, purity, and potency standards. With global demand for incretin-based therapies already outstripping supply for tirzepatide (Mounjaro/Zepbound) and semaglutide (Ozempic/Wegovy), Lilly is building retatrutide’s manufacturing capacity ahead of potential regulatory approvals.

What CMC innovation means for patients

CMC encompasses the entire process of making a pharmaceutical product — from synthesizing the active peptide to filling injection pens under sterile conditions. For retatrutide, a synthetic peptide with specific amino acid modifications that extend its half-life to enable once-weekly dosing, the manufacturing process is more complex than for single-receptor agonists. Lilly’s CMC work has focused on scaling production while maintaining the precise molecular structure required for triple-receptor activity.

The company’s manufacturing strategy for retatrutide includes both internal capacity expansion and contract manufacturing partnerships. Kopach’s comments suggest Lilly is confident it can avoid the supply shortages that have plagued earlier GLP-1 drug launches — a critical consideration for a treatment that could eventually serve millions of patients.

Canadian Context

Canada has experienced the GLP-1 supply shortage acutely. As of mid-2026, semaglutide and tirzepatide remain on Health Canada’s Tier 3 drug shortage list, with intermittent availability at Canadian pharmacies. The shortages have been particularly difficult for patients with type 2 diabetes who rely on these medications for glycemic control, not weight management alone.

Lilly’s emphasis on manufacturing readiness for retatrutide has direct implications for Canadian access. Under Health Canada’s regulatory framework, a New Drug Submission (NDS) includes detailed CMC data — the Chemistry and Manufacturing section of the submission is reviewed for process consistency, quality controls, and supply reliability. A robust CMC package can expedite Health Canada’s review by reducing information requests and follow-up rounds.

Additionally, Canada’s pharmaceutical supply chain relies heavily on imports. The ability of a manufacturer to commit to North American supply volumes affects whether a drug will be allocated to the Canadian market at launch or whether Canadians will face extended wait times. Lilly’s CMC preparations, if they deliver on promised capacity, could mean earlier access for Canadian patients following Health Canada approval.

Canadian clinical trial participants in the TRIUMPH program — including 22 sites in TRIUMPH-OUTCOMES and 12 sites in TRIUMPH-5 — have contributed data that supports the global regulatory dossier. Their participation helps ensure the evidence base reflects diverse populations.
